In practice, analysts do not need to be told that these considerations should in clinical work only be an explanation of last resort, that the search for psychological causes must remain paramount. But there is another set of factors that must not be neglected in this context. Granted that variations in congenital equipment exist, the response of the selfobject milieu to congenital shortcomings must by all means be taken into consideration, and any resulting psychopathology-for example, an excessive tendency to develop panic states or an inability to curb anxiety-must be explained not only in terms of the congenital defect, but also with regard to the failure of the selfobjects to respond adequately to an especially trying situation.
It is by no means necessary for analysts to discard this nineteenthcentury ideal to which Freud and his contemporaries subscribed. In a specific, circumscribed sense it is as valid a guide now as it was then: if we want to see clearly, we must keep the lenses of our magnifying glasses clean; we must, in particular, recognize our countertransferences and thus minimize the influence of factors that distort our perception of the analysand's communications and of his personality. But this purely cognitive approach to the problem of scientific objectivity in our field, however valid within certain limits, is inadequate.
How does self psychology look at the phenomena presented by agoraphobia? The answer to this question is as simple as it is significant. As I said before: what traditional analysis looked upon as secondary and peripheral self psychology sees as primary and central. This follows from the fact that self psychology considers the structural and functional deficiencies of the patient's self as the primary disorder and focuses its attention on them, whereas traditional analysis saw the content of conflicts as the primary disorder and focused on it.
